    ICT Excellence Awards 2010

Safe Mobile Care is an end-to-end telehealth solution that empowers patients to manage their condition at home while allowing healthcare professionals to monitor and prescribe for their condition remotely, providing a better patient experience.  Examples include diabetes, asthma, heart disease and many more conditions.

The innovation is truly mobile, allowing patients to maintain their independence without being reliant on ‘black boxes’ which sit in their home.  Using a mobile phone device, the innovation is tailored (i.e. prescribed) uniquely for each patient and allows them to enter daily readings when it is convenient for them by pocket Bluetooth medical devices.

The solution remotely captures medical data from patients and analyses incoming information to identify patients who are experiencing (or predicted to experience) abnormal clinical episodes.  This allows the healthcare professional to proactively target those patients who need the most assistance without compromising their level of care over other patients. The solution gives independence and mobility to those suffering long term conditions and provides a means to empower the patient to manage their condition by bridging the communication gap between patient and clinician.
